Thank you for completing the Report of Teaching Experience for Salary Rating Purposes.

The Metropolitan Public Schools and Tennessee Department of Education require that the following information be completed on the Report of Teaching Experience for Salary Rating Purposes.

1. Complete the name of the school/school system/city/state. Do not report public and nonpublic school experience on the same form. If experience is from more than one school, each school must be reported on a separate form.

2. Enter each school year on a separate line, July 1 - June 30. Example:

07/07/98 - 06/30/99
07/01/99 - 06/30/00

3. Give the beginning and ending month, day, year and the total months and days served in position. The school year must have a beginning and ending date.

4. Indicate if time served is full-time, and what percent of time, if part-time.

5. Give the name of the accrediting agency. Agencies recognized by Tennessee Department of Education are on the second page of the form.

6. Report of Teaching Experience should be signed by a school official, give title and address.  

If the above information is returned incomplete, it will delay processing prior teaching experience, and Tennessee Department of Education will not recognize the experience.
