Conduct Research (Elements of an Informed Consent Form)
 
  • It should be in simple language, so that the person reading can easily understand it.

  • The name of the researcher should be at the top with the institutional affiliation of the researcher.

  • Describe the procedure to be used, the purpose of the study, and how long the study will take.

  • Describe any potential risks to the participant, if any.

  • Describe in detail how you will protect the participant. This description should include your method for separating the name of the participant from the data obtained from that participant and how you will secure the identifying information. Please note that anonymous data are those never related to any names, while confidential data are those coded with a series of numbers and letters that can be identified with a particular individual in some point in time.

  • State that the participant can stop participating in the study at any time or can refuse to answer any question for any reason.

  • List any benefits coming to the participant. It can be stated that there are none but that the study contributes to knowledge about education.

  • Include a statement that participation in a study at a school does not compromise school services.

  • A copy of the consent form must be included with the proposal when it is submitted or it will not be considered.
